By PAULA SPAN

When your aging parent is cycling from hospital to rehab to home and around again, when you’re sitting in an E.R. with him wondering if he really ought to be there, what you really want is a friend in the business.

You want someone who knows the health care ropes and the players, and can tell when a senior is being undertreated because of medical ageism ("What do you expect at your age?") or overtreated because ordering expensive tests takes so much less time than sitting down with a patient and listening.

You’d like this friend to be honest — no, blunt — in his medical counsel. He would explain, wryly but actually, how to get the most out of what he calls the "incredible shrinking office visit." He’d acknowledge that doctors can drive you crazy by deciding that an elderly patient is ready for discharge from a hospital without any sense of where they’re headed afterward. Then he’d tell you how to respond.

Dr. Mark Lachs has volunteered to be our friend in the business. An insider, as director of geriatrics for the NewYork-Presbyterian Healthcare System, he’s surprisingly candid about the pitfalls and shortcomings of on temporary medicine, especially for old people.

Yet his new book, "Treat Me, Not My Age: A Doctor’s Guide to Getting the Best Care as You or a Loved One Gets Older," goes beyond analyzing the systemic problems of physicians who aren’t trained to treat the elderly and hospitals designed to heal patients with a single problem, not multiple illnesses.

What is memory loss?

Memory loss is something we all experience in life. We forget familiar names, we cannot remember where we left our wallets and purses the previous evening, and we can’t remember everything needed at the grocery store without having a list. This type of memory loss is perfectly normal and as we age, such mild forgetfulness may start happening more and more.

However there are also times when memory loss is not attributed to aging. For example, memory loss is one of the first signs of Alzheimer’s disease. Memory loss can also happen after suffering a traumatic experience. It can happen if you suffer from depression or are lonely, sad, worried and even when you’re bored. It can result from dehydration, or after falling and hitting your head, or because of the way different medications you’re taking interact with one another. Excessive use of drugs and alcohol can also be factors in memory loss.

Good news is that after age 110, chance of death does not increase. Bad news is that it holds steady at 50% per year.

People today are living substantially longer than their parents’ generation, not because aging has been slowed or reversed, but because they are staying healthier. A demographer writes on the longevity phenomenon in the March 25 edition of Nature and wonders how we can keep in going.

People in developed nations are living in good health as much as a decade longer than their parents did.

"We’re living longer because people are reaching old age in better health," said demographer James Vaupel, author of a review article appearing in the March 25 edition of Nature.

September is Healthy Aging Month and we wanted to share this article about aging from the ARA. There are so many things we can do to start aging gracefully and discussing our concerns candidly with our Doctor is very important. Taking care of ourselves now can help us take care of others as the population gets ready to explode with aging Baby Boomers. Don’t forget Grandparents Day on September 12th and remember, for the best in healthcare and assistance, visit www.inhomecaresandiego.com.

Tips For A Healthier You At Middle Age
(ARA) – With the popularity of movies like Sex and the City and the revival of the 50-year-old fashionista, the 42.9 million baby boomer women in America are approaching aging with confidence and style. While talking about many sensitive topics is no longer off-limits to this vocal group, there still remain several embarrassing health topics that many women will not even discuss with their doctors, let alone with their girlfriends. As a result, they suffer through these manageable health issues in silence rather than finding the solutions they need.

It is important for women to be direct with their doctors when talking about all health issues, even those that may embarrass them. Anne Abernathy, a six-time Olympic luger who now serves as an advocate for women’s health, shares tips for being tenacious when it comes to your health:

Talk to your doctor: You may be embarrassed by a change in your body, but when you talk about it with a friend, family member or doctor, you can find helpful ways to deal with it. You may even be able to provide helpful advice to others experiencing the same thing. Facing your health condition head-on and learning how to manage it will help you to move on and live your best life.

Educate yourself: Learn about the kinds of changes you can expect as you age and take some preventative measures to help you avoid or lessen the symptoms of some of them. There are a lot of great sources online where you can learn from experts, as well as others who have first-hand knowledge of the issues. Information is the best tool you can have as your own health advocate.

Navigate your options and solutions: There are a lot of options available to help you manage and prevent certain conditions and ailments. For instance, vitamins to help prevent bone loss or discreet thin pads or underwear for women who have bladder control issues. Talk with your doctor to find out what supplements or products might be right for you.

Achieve a healthy life: In the end, you just want to get on with living your life and achieving your goals. No health issue should keep you from doing that. When you follow these tips and learn how to manage your condition, you will find yourself with more options, less limitations and more confidence to do what you love most.

Many of these health problems are extremely common, like bladder-control issues, which affect more than 25 percent of women older than 40. According to a survey conducted by Harris Interactive and SCA, makers of TENA, the number one brand of bladder protection products worldwide, nearly 40 percent of women with bladder-control issues have never discussed the condition with anyone. As a result, nearly a third of all women who experience leaks do not know how to deal with the problem.

"Throughout my life, I have not been afraid to tackle the many obstacles that have stood in my way," says Anne Abernathy. "Having experienced embarrassing health conditions such as bladder-control issues first hand, I know that for many women, the thought of admitting to the condition can be more stressful than racing down a luge track at 90 mph. That’s why I teamed with TENA and Healthy Women to launch the ‘Be TENAcious’ program to help women start the conversation, beginning online at www.betenacious.net. Many women experience the same issues in their lifetime, but we cannot benefit from each other’s experiences if no one is talking about them."

Registered nurse Beth Battaglino Cahill, executive director of HealthyWomen agrees. "As a nurse, I often have to coax information out of patients about health issues they find embarrassing. When women are candid about their health, they often find the problem they have is very common and managing the issue becomes a lot easier."

September is Healthy Aging Month! As we age, one of the most important things we can do for our bodies is eat healthy. WebMD has some great advice on what you can eat to keep the pep in your step! Read the excerpt here, and the full article at www.webmd.com.

The Anti-Aging Diet
Can what you eat help you age gracefully?
By Kathleen M. Zelman, MPH, RD, LD

It seems we’re all trying to find the "magic bullet" that delays the natural aging process. Put an antiaging label on most any product, and it flies off the shelves.

Yet if you’re trying to look your best without going under the knife, a secret ingredient might be right under your nose. Some experts say one answer to aging gracefully can be found in the grocery store — in fruits, vegetables, green tea, and a host of other healthful foods that are rich in antioxidants and other potentially age-deterring compounds.

What Is Aging?

Of course, the signs of aging include not only wrinkles, but also memory loss, decreased brain function, and an increasing risk for chronic diseases such as heart disease, osteoporosis, and cancer. Healthy aging is also defined as living a longer, healthier life. And many studies have documented the link between a healthy diet and prevention of age-related or chronic diseases.

Adopting a healthy lifestyle that includes regular physical activity, adequate rest, avoiding tobacco, and a diet full of healthy foods and beverages can be the best defense against aging.

"Dietary choices are critical to delay the onset of aging and age-related diseases, and the sooner you start, the greater the benefit," says Susan Moores, RD, a spokesman for the American Dietetic Association.

Antioxidants and Inflammation

Some foods and beverages contain powerful substances called phytonutrients that some believe are capable of unlocking the key to longevity. Phytonutrients, which are members of the antioxidant family, gobble up "free radicals" — oxygen molecules that play a role in the onset of illnesses such as heart disease, cancer, osteoporosis, and Alzheimer’s disease.

As we age, we become more susceptible to the long-term effects of oxidative stress (a condition where the body basically has too many free radicals) and inflammation on the cellular level. The theory is that antioxidants and other age-defying compounds help cells ward off damage from free radicals and minimize the impact of aging.

Beyond antioxidants, some other compounds in foods can affect aging. They can be classified according to their impact on inflammation at the cellular level, experts say.

"All foods fit into three categories: pro-inflammatory, neutral, or anti-inflammatory," says dermatologist and best-selling author, Nicholas Perricone, MD.

Perricone says you can help to slow aging at the cellular level by choosing foods that are anti-inflammatory and rich in antioxidants.

"Age-related changes may be reversed by consuming foods and beverages that are rich in a variety of compounds, including antioxidants, and are anti-inflammatory, such as cold-water fish and richly colored fruits and vegetables," he says.

On the other hand, foods classified as pro-inflammatory can accelerate aging, Perricone says.

If "we eat large amounts of saturated or trans fatty acids, sugars, and starches, insulin levels surge and trigger an anti-inflammatory response and accelerate the aging process," says Perricone.

While the benefits to eating healthy are many, Perricone notes that diet is certainly not the only factor that affects the aging process.

"Stress, hormones, ultraviolet light, and a weakened immune system also contribute to aging," he says.

Still, making smart lifestyle choices are within your control, and are among the best things you can do to help prevent disease and retard aging.

Medicare is changing its supplemental policies: how will this affect you?

Understanding the changes to Medicare’s supplemental policies

Important changes to Medicare supplemental policies — also known as Medigap plans — took effect in June. Millions of older Americans buy the plans each year to help pay medical bills.

Vicki Gottlich, an attorney at The Center for Medicare Advocacy (medicareadvocacy.org), described the new developments. Her remarks have been edited.

Q What are Medigap plans and how do they work?

The Healing Power of Therapy Animals in Bostonia CA

ARA) – When Sunny, a 3-year-old golden retriever, began working for San Diego Unified School District last spring, one of her first students was a fourth-grader with cerebral palsy, mobile only with the aid of a walker.

Adaptive physical education teacher Andrea Bazer says that as soon as this young student met Sunny, she was eager to take Sunny for a walk — first with the help of her walker, then with Bazer helping, and finally without any assistance. This transformation took several months, but once the student knew she could walk Sunny without the walker, she realized she didn’t need a walker at all.

"She walks all over school now," says Bazer, who has worked as an adaptive physical education teacher for the district for six years. "It’s amazing to see. Sunny gets the kids to do many things that they won’t do for me. When they’re working with Sunny, they forget they can’t do things."

Therapy animals like Sunny are highly trained and play very active roles in the educational or therapy program they’re involved in. Sunny helps by playing fetch and other interactive games with the students. Children in therapy with horses benefit through interacting with the animals and riding them. A client must be confident to lead and/or train a therapy animal, and this provides an opportunity for growth.

Today, animals help out in educational and health care settings so often that the wide array of roles they play can be a bit confusing. For example, within the ranks of assistance animals, there are therapy dogs like Sunny and also visiting animals, which visit the sick in hospitals or nursing homes, providing a warm, soft head to pat.

Bazer works with about 45 preschool to sixth grade children with disabilities at five schools, and she said that Sunny is an asset in her class. Sunny recently helped Bazer achieve a breakthrough with a preschool-aged child who was refusing to open up when faced with a stranger – Bazer. "The child’s first word during the assessment was ‘dog,’ as soon as he saw Sunny," Bazer explains.

Sunny is a hard worker as well; she works 40 hours a week at schools, and then Bazer regularly takes her to the Naval Medical Center in San Diego to give a psychological boost to wounded soldiers receiving treatment.

Dr. Hayden Sears, vice chairman of the American Veterinary Medical Association’s (AVMA) Committee on Human-Animal Bond says the impact animals can have on hospital and nursing home patients, people with disabilities, children and the elderly in therapy situations is remarkable to witness. Dr. Sears worked with therapy animals prior to becoming a veterinarian.

"I have also done a lot with horses and people," Dr. Sears explains. "People with multiple sclerosis, people with broken backs, all the way down to children with cerebral palsy, really benefit from working with a therapy horse."

Dr. Sears says that while most people associate therapy animals with a psychological boost for patients, the impact can also be very physical. For example, horseback riding is known to stimulate the nervous system and offer strengthening and mobility therapy for the disabled. Research consistently shows that therapy animals are helpful with multiple conditions, including heart disease, substance abuse, schizophrenia and dementia.

The active ingredient in animal therapy is a little-understood concept called the human-animal bond. The AVMA has officially recognized the existence of the human-animal bond since 1982. And the AVMA states that this bond has existed for thousands of years. "Interactions with animals can provide emotional and physical health benefits for diverse human populations, including the elderly, children, physically disabled, deaf, blind, emotionally or physically ill, and the incarcerated," AVMA policy states.

Therapy animals have a long history, as well. Florence Nightingale recommended the use of a small pet to provide companionship to the sick, and ancient Greeks believed dogs had healing powers.

While most therapy animals are dogs and cats, therapists have found success using chickens and even small ruminants like goats.

"There have been therapy programs that have worked with cattle," says Carol Davis, executive director of Paws’itive Teams, a California organization that trains both therapy and service animals. Paws’itive Teams is the organization that trained Sunny.

Davis says that over the past decade she’s seen the demand for her animals increase and more diversity in the work these animals are asked to do. Today, therapy animals trained by Paws’itive Teams work with foster children during evaluation sessions and with children who have been victimized.

"We have one dog that works at a county courthouse with children who have been abused and will have to testify against their abuser," Davis says. "In some cases, the judge will allow the children to take the dog with them onto the stand for support."

The AVMA policy on therapy animals suggests that the relationship between therapy animals and their human clients must be mutually beneficial, explains Dr. Emily Patterson-Kane of the AVMA Animal Welfare Division.

"People experience great benefits from living or working with therapy animals, and we must make sure that the animals benefit from working with us," Patterson-Kane says. "The real power of the human-animal bond is that people and animals can make each others’ lives richer and more meaningful."

For sandwich generation, planning ahead is key to good elder care.

How do you take the car keys away from a father who taught you to drive? When did he go from wise council to frail, elderly man?

Unfortunately, the What to Expect When You’re Expecting book series on parenting doesn’t have a volume on parenting your parents. If anyone thinks dealing with aging parents is easy, they’re deluding themselves. It is often one of the most difficult challenges people face during their adult lives–and one for which they’re least prepared.

The consequences of inaction, meanwhile, can be severe. Many adult children don’t understand the complexity of the problem. Why would their parents resist setting up a power of attorney? Will they have to be dragged kicking and screaming to a senior facility? The answer all too often is "yes," even well after it has become painfully apparent to others that they are no longer capable of handling their own affairs.

The fact is many elderly people don’t see themselves as elderly and hate being around other old people. To them, moving to a senior facility involves making a move that they feel they can never undo; they are moving in their minds from independence to dependence. Hence the kicking and screaming.

Deciding On Care For Elderly Parents

TWO years ago my father, then 83, became very ill. Until then, he had been living alone in a pleasant one-bedroom apartment on the Hudson River, an hour’s drive from my home in Brooklyn.

After a couple of months in the hospital it became clear that my dad, Harvey Alderman, could not return to solo living. He was fragile and forgetful, and there was no way he could keep track of the 14 or so pills he had to take each day.

But where would he go — and how would we pay for it? Could he stay in his apartment if he had regular visits from an aide? Or should he go to an assisted-living facility where there would be more services available for him?

So began my family’s crash course in caring for an aging parent in declining health.

If you’re in this predicament, you know already there is no simple answer. Older people each have unique medical and emotional needs. And finances often dictate how far you can go in creating the ideal situation for them.

That is what Linda Chase, a lawyer in Reston, Va., realized after running the numbers on what it would cost for home care for her mother, who has dementia and needs round-the-clock attention.

“We couldn’t afford private home-health care, so the only option for us was assisted living in a facility with dementia care,” Ms. Chase said.

Below, I offer guidelines and considerations that can help you make an informed — if not always easy — decision about what type of housing will support your parent’s needs, without bankrupting the family in the process.

And note: While the following discussion refers to a single parent who lives alone, many of the considerations would also apply to an elderly couple who are each in declining health.

Aftercare Tips for Patients Checking Out of the Hospital

According to a study published last year in The New England Journal of Medicine, one in five Medicare patients returns to the hospital within 30 days of being discharged. The problem is an expensive one: in 2004, these readmissions cost Medicare $17.4 billion dollars, the researchers also found.

IN mid-March my 85-year-old father checked into a prominent New York City hospital for a scheduled operation. The procedure, to remove a cancerous tumor from his thigh, went well, and soon he was sent home.

But three days later, unable to cope with a complicated wound care regimen, he landed back in the hospital.

My father had become part of a notorious trend. Discharge from the hospital is a critical point in a patient’s recovery, particularly for older people with chronic conditions. The process is supposed to be carefully planned, but instead it often is rushed and poorly coordinated, resulting in complications that send patients back to the emergency room.

According to a study published last year in The New England Journal of Medicine, one in five Medicare patients returns to the hospital within 30 days of being discharged. The problem is an expensive one: in 2004, these readmissions cost Medicare $17.4 billion dollars, the researchers also found.

Hospital stays certainly are shorter now: the average stay was 4.6 days in 2007, down from about 5.7 days in 1993. But the readmissions problem is not simply the result of compressed care, experts say.
